Whole-Health Support at Every Visit

Whole-person care means knowing each patient’s health issues, not just the reason for their visit. Regularly checking chronic and complex conditions helps you see each patient’s disease burden. This supports stronger, more coordinated care.

Consider implementing these workflow improvements:

  • Reserve longer visits for patients with multiple chronic and complex conditions, including acute care.
  • Add any unaddressed conditions during pre-visit prep. Even an acute care visit is an opportunity to spot issues that may affect care, referrals, or patient education.
  • Keep the problem list up to date before and after the visit. Work with a physician clinical reviewer to resolve conditions using medical record documentation. A current list helps the care team make treatment and care-coordinated decisions.
  • Document the conversation about their conditions. Make sure the medical record reflects monitoring, evaluation, assessing/addressing, and treating (MEAT). Using ambient AI during the visit can help you capture clear details faster, and many providers are already using it.
