Easy Ways to Strengthen Patient Care

We partner with you to help members navigate their care. This month, we’re launching two priority campaigns to support clinical care and improve outcomes: heart health and emergency department (ED) follow‑up.
Why it Matters
Member engagement helps patients follow their care plan, overcome barriers, and stay connected between visits. Our outreach follows Healthcare Effectiveness Data and Information Set® (HEDIS) care guidelines. This partnership helps close care gaps and improve health outcomes.
What Patients Get
- Heart health: Encourages patients to stay up to date on heart checks, understand their care plan, and build healthy habits.
- Emergency department follow-up: Helps patients schedule a follow‑up visit after an ED visit (within 7 to 30 days). We also clarify discharge instructions and help with common barriers like transportation, supporting a safe transition back to provider care.
